What to Report

Some items to report include the following:

• Academic concerns that include absenteism and performance issues

• Questionable or suspicious acts or behavior

• Knowledge of illegal actions, assaults, and/or drug or alcohol sale or usage.

• Expressions by others of feelings of depression, reduction of self-worth, and/or uselessness.

 

 FOR FACULTY/STAFF ONLY:

This report form replace the “Early Alert” form.  You may choose to submit this report anonymously or provide your contact information.  The report is submitted via a secure server housed on the “Red Flag” system.

All submissions will be reviewed and routed to the appropriate office or resource; who will then in turn contact the student.  This is consistent with how Early Alert submissions were handled.

Note: The Username and Password should not be your SOAR information.  If you want to submit an anonymous report, please choose a username and password that is easy to remember.

If you are not submitting an anonymous report, your username may be your USM email.

There is separate “login information” for each incident. This is to ensure that each report is handled separately and for tracking purposes.
